  Pretty soon these Outcome assessment forms will be required as part of our documentation.....to justify care. If ReportMaster can add this in the program then it would be awesome!

  This is a great idea. NDI, Oswestry, Roland, Croft's Whiplash grading etc. are all important.

  Yes. I totally agree. Having these Outcome assessment forms built-in would be awesome. It should be available at least for re-exams. These forms are crucial if you want to prove medical necessity.

  Specifically the Oswestry and also the ParQ. Having a Review of Systems could also be a great benefit and it to be able to generate a problem list with dates and we could always add onto this as we see the patient throughout the years.

  It is extremely important that we use outcome assessment tools to justify our treatment plans, and to assess the need for future care. I would love to see outcome assessment forms for neck disability, Oswestry Low Back, upper extremity, lower extremity, headaches, & shoulders. If it is not practical to put the entire form on, then at least a page where we can enter the value from the form the patient fills out in person, with the ability to show future results on subsequent questionnaires so that we can demonstrate imporvement etc.

  Agreed. These are the standard forms that are widely accepted and are invaluable in PI or work comp cases. I design my own autocodes and just have to make slight changes now, but it would be nice to just have it. In addition,if you can print out just the outcome assessment report it is billable. This IS an important suggestion.
